Certified that the documents submitted by the applicant have been scrutinized and only those documents which are found to be correct have been taken into consideration. It is further certified that the above assessment has been done as per policy guidelines applicable for the year of award.
Certified that particulars of the applicant and sports achievements for the years 2016-17 (from 01-04-2016 to 31-03-2017) have been checked from the official record. Wherever variations has been observed, modifications have been made initialled by me. Accordingly these are verified.

I have never been penalized for any unfair practice like age fraud/ positive in doping test, etc. All documents attached including Sports Certificates, Haryana Domicile Certificate, Date of Birth Certificate, etc. are true & correct.

** Certificates of First/Second/Third position/Participation of the sports persons in the recognized game as per rules.
** Certificate/Documentary proof of having represented Haryana in the games as per rules.
** Attested Copies of Sports Achievements.
** Date of Birth Certificate from Municipal Committee/Matriculation Certificate/Registrar Birth & Death.
** Haryana Domicile Certificate
** Three passport size photographs of applicant
** Copy of PAN Number.
** Copy of Aadhaar Card
** Copy of Bank Account with complete details as per application form
** Declaration by Applicant on stamp paper of Rs 10/- Regarding Doping and Sports Achievements

In case of Awards for School/University Games concerned Head of the Institution will certify the required documents
