Application form for sanction of financial assistance to the scheduled caste students who have passed inter and final examination of ica/icma/icsi (ca/kcwa/acs) courses

1.Aadhaar. No and PANCARD. No. (Compulsory)
2.Name of the student (In block Letters)
3.Name of the Father/Guardian
4.Permanent Address/ Mobile No./ Email ID
5.Caste/Sub-Caste (Certified copy of the caste certificate issued by Tahshildar should be enclosed) /RD no. if available
6. Institute of Charted accountants of India/Registered No. Registration Date
7.Name of the course to which admitted
8. Fee paid (copy of the fee receipt should be enclosed)
9. Bank details of the student (Name of the bank, Branch, SB Account Number AND IFSC Code. (Compulsory)
10. Exam passed, enclosed pass certificate
11. Student studying in which course
12. Percentage of marks (%) in graduation (enclose copy of degree marks card)
13. Post Graduation certificate (If applicable)
14. Income of family from all sources

The following certificate must be endorsed by authorised signatory of the Institute of Chartered Accountants of India or Relevant Institution.

CERTIFICATE :

He/She has registered to _____________course during _____________ year in this Institution, He /She has paid Rs.__________ towards fees at the time of registration/admission, The paid fees is not reimbursed from scholarship or other sources.

Incentive to meritorious SC Students:
As per GO, incentive of Rs.50000/- will be paid to SC Students who have passed Inter exams and Rs.100000/- will be paid who have passed Final exams in CA/ICWA/ACS.
